Managing Multiple Medications

presentation on Polypharmacy.
Many people—especially older adults and those with ongoing health conditions—take several
medications each day. While medications can be lifesaving, taking too many medicines or the wrong
combination can sometimes cause side effects, confusion, or harm. This presentation, led by a
pharmacist from NewYork Presbyterian Brooklyn Methodist Hospital, in collaboration with
Good Neighbors of Park Slope, will help community members better understand
polypharmacy—what it means, why it happens, and how it can affect everyday health.
Meggie Chan, PharmD, BCPS, BCCP, CACP has over 10 years of experience as a clinical pharmacist in critical care cardiology at NYP Brooklyn Methodist Hospital. Her professional interests include acute coronary syndrome and anticoagulation.
Eileen Chung, PharmD, BCPS, BCCCP, BCPPS has over 10 years of experience as a clinical pharmacist in pediatrics at NYP Brooklyn Methodist Hospital. Her professional interests include critical care and medication safety.
Kay Takamura, PharmD, BCPS, BCPP has five years of experience as a clinical pharmacist in psychiatry at NYP Brooklyn Methodist Hospital. Her professional interests include long-acting injectables and deprescribing.
